What happens after I send this form?
We call back within one business day. Your answers decide who calls: the state you are in tells us which clinicians are licensed to see you, and the type of therapy you picked tells us who on the team does that work. If you are not sure what you need, say so and we will sort it out on the call.
Can I book a consult without filling out this form?
Yes. If you would rather just pick a time, book a free 15-minute consult and we will talk it through live. The form is there for people who would rather write it down first, or who want us to call them.
What if I have Medicare or Medicaid?
We do not accept Medicaid or Medicare.
How is insurance submission handled?
If you would like to use your insurance, as a courtesy we submit the claim to your insurance provider for reimbursement directly to you. We recommend calling your insurance provider to confirm whether you have out-of-network benefits.
All payments are due at the time of service. We accept credit cards only and keep a card on file for every client.
